Community Service and Personal Growth Verification Form

The University Honors Program, in accordance with its mission statement, requires students to participate in service learning and extracurricular activity.  Honors students are asked to complete a minimum of ten hours of community service each academic quarter.  Most students become involved in an activity they find highly rewarding and complete many more than ten hours of service.  The Community Service and Personal Growth Verification Form is a means by which the University Honors Program can accurately track and evaluate the activities completed by its students.  If you have any questions regarding the use of this form please contact the UHP Office.
